Forum Description
Epidemiological evidence indicates many non-reproductive cancers are more frequently observed in men than women. Emerging evidence from immunotherapy clinical trials also suggests that treatment response and therapy-related adverse events are different between the sexes. A Forbeck Forum focusing on hormonal and chromosomal bases of observed differences in cancer prevalence and treatment response between males and females could accelerate research in this understudied field. Understanding the unique pro-tumorigenic mechanisms in each sex could facilitate the development of more effective anti-cancer therapies. This timely meeting could further highlight the need to consider patient sex as a biological variable, especially given the increasing use of immunotherapies.
Despite growing appreciation for sex differences in cancer prevalence, outcomes, and treatment response; molecular mechanisms contributing to this discrepancy remain poorly defined. There is a pressing need to stimulate collaboration across research groups and clinicians to define unique aspects of tumorigenesis to develop more effective therapies that account for sex as a biological variable.
Forum Summary
Decades of population health data reveal that cancer incidence and mortality is greater in men compared to women in nearly all tumors of non-reproductive tissues. In the past few years, there has been an explosion of research investigating how features of an individual’s sex (sex chromosomes, sex hormones, social determinants of health, etc) contribute to this disease risk difference. The purpose of this meeting was to bring together experts from across the country who are thinking about this question from unique perspectives.
The meeting was attended by 12 investigators. Two scholars were initially registered; however, due to changes in federal funding and university travel restrictions, these scholars were unable to join us. The group of investigators spanned the spectrum of the investigation of sex differences in cancer. Dr. Quinn Ostrom from Duke University presented epidemiological observations from population health studies on correlations between allergies, autoimmunity, and brain cancer risk. Notably, whereas hay fever allergies lowered overall risk of brain cancer for both men and women,autoimmunity (specifically RA or SLE) was only protective in female against high grade gliomas. Dr. Hannah Carter (University of California, San Diego)also leveraged population health data to generate polygenic risk scores associated with response to immunotherapy and found that these were different between men and women. Notably, female tumors had fewer neoantigens compared to male tumors.
These observations were complemented by mouse model studies by Drs. Justin Lathia (Cleveland Clinic), Elise Alspach (Saint Louis University), and Kyuson Yun (Houston Methodist) who outlined sex differences in innate and adaptive immune responses (platelets, microglia, astrocytes, and Tcells) in brain and bladder tumors. In both models, female tumors-whether in the brain or the bladder-were enriched for white blood cells that are associated with better overall prognosis, highlighting the differences in immune-cancer interactions in males vs females. On this topic, Dr. Defne Bayik (University of Miami) shared a key sex difference in a subset of these innate white blood cells called myeloid derived suppressor cells (MDSCs), in their expression of neurotransmitter receptors. This signaling difference informed sex-specific immunosuppressive mechanisms. Other attendees at the meeting including Drs. Laura St. Clair (Johns Hopkins University) and Joseph Ippilito (Washington University St. Louis) revealed key metabolic differences in immune and tumor cells from male and females. The key takeaway, that famine favors female, became a recurring catch-phrase throughout the meeting!

Dr. Katie Basham from the University of Utah challenged many of the paradigms discussed at the meeting by presenting data on adrenal cancers which are twice as common in females compared to males. Her research focused on sex differences, mediated by androgens rather than glucocorticoids, in regulating macrophage trafficking and function in male and female adrenal tumors. Also leveraging the immune-modulating function of androgens, Dr. Amy Moran (Oregon Health & Science University) discussed an array of mechanisms by which testosterone blockade can activate the immune system-from thymic regeneration to enhanced innate and adaptive immune responses and synergy with immunotherapy.
Finally, in a fascinating lecture from Dr. Sri Viswanathan (Dana Farber Cancer Institute), the group learned about X-linked genetic factors that underlie sex differences in cancer. Leveraging a unique gene fusion between an X-linked gene (TFE3) with a gene on an autosome (non-sex chromosome), he revealed a sex-chromosome mediated mechanism of kidney cancer (renal cell carcinoma). Interestingly, he raised the possibility that in XX individuals, if the ‘inactive’ X-chromosome forms a translocation with a gene on an autosome, such as a tumor suppressor gene, then it’s possible to have X-linked gene silencing of non-X genes. Dr Viswanathan’s talk was the final one of the meeting, reducing some sex differences down to sex chromosomes!
The group finished the meeting with a brainstorming session on defining what we mean when we talk about sex differences. We discussed the importance of specifying hormone-mediated or chromosome-mediated differences,observation based on age, and/or simply sex organs displayed by an organism. New friendships and collaborations were forged and all agreed that this intimate‘think tank’ meeting was one of the best we’d ever attended!

Denver International Airport (DIA) is approximately 30 minutes from the venue.
- Arrival day of the forum at approximately 1:00 PM, 3:00 PM and 5:00 PM

IMPORTANT INFORMATION ABOUT GROUND TRANSPORTATION
To maximize funding for our Forums, the Foundation does not reimburse ground transportation costs outside the designated shuttle pick-up times. If you arrive after the final shuttle, or do not wish to wait for or take a scheduled shuttle, you will be responsible for arranging and covering your own transportation to the Forum venue.

As you make your plans please remember that we are a nonprofit organization dependent on donations and volunteers. So please note what we do and do not cover.
What the Foundation Pays For
- ACCOMMODATIONS for the nights of the Forum only. People traveling from oversees or far away may arrive 1 night early to adjust for jet lag or to navigate longer travel times.
- MEALS are provided by the foundation during the forum.
- A TRAVEL ALLOWANCE FOR AIRFARE will be set based on an economy ticket from your home airport. If you have travel before or after that is fine. We do not increase the allowance for other travel plans.
- The Foundation provides SHUTTLES TO/FROM THE AIRPORT to the meeting venue at designated shuttle times.
- You can select not to utilize Foundation arranged transportation at your own expense when completing the travel form.
What the Foundation does not cover
- We do not cover upgrades of any kind.
- Travel Allowance will not be increased due to late booking or changes.
- We do not cover incidental costs during your travel. So please do not send your food receipts.
- We do not cover additional nights at the venue outside of the meeting dates.
- We do not cover transportation outside of the designated shuttle times.
- We do not reimburse for home side airport transfer (to/from your home/office to your airport).

Abstracts
Abstracts are due 30 days prior to the start of the meeting to allow enough time to prepare the meeting book.
The abstracts should be only one or two paragraphs outlining the theme of your presentation and should reflect the objective and spirit of the meeting (see above). Abstracts will be circulated about one week before the meeting. The meeting organizer will start requesting them a month before the meeting.
Forum Structure
The structure of the forum has been developed over years of experience.
- Participants have approximately 45 minutes, depending on the number of participants, for their presentation and discussion. The presentation is meant as a conversation start and should last about twenty minutes briefly covering background information and areas that are new or need further input. This should be structured in such a way as to lead to a lively discussion. Participants are encouraged to interrupt to ask questions or start discussions.
- A MAXIMUM of 2 slide equivalents per presentation is allowed (Power point slides should not contain more than one graph or gel per slide and no more than 5 bullet points to stress the points being made by the presenter.) We appreciate cooperation with the spirit of this guideline.
- A Whiteboard and markers will be available to supplement your digital slides.
- Everyone is expected to actively participate in every session and discussions.
- The time spent at the forum is relatively short, so please be familiar with papers received prior to arrival.
- It is very important that you commit to all sessions of the 2 days of the forum.
Forbeck Scholars Participation
Scholars are selected for each Forbeck Forum. These are outstanding junior clinical or post-doctoral fellows selected based on the quality and relevance of science.
- Scholars present for 30-45 minutes, depending on the number of participants
- The same presentation rules apply for scholars
- After the Forum you are selected to attend, you will attend three years of Scholar Retreats held in Lake Geneva, WI. If you attend a Fall Forum, you will attend the Spring Retreat. If you attend a Spring Forum you will attend a Fall Retreat.
- Scholars are selected by the Foundation Scientific Advisory Board and peer reviewers selected from past Forbeck Scholars.
